If you are unable to find a desired time, please contact the office at (303) 563-2760 and the receptionist will be happy to accommodate you.

All patients must have an MRI performed within the last 3 months to schedule a new patient appointment.

To reschedule an appointment, call us directly at (303) 563-2760. 

In order to expedite your visit, please bring the following items:
*A list of current medications
*Insurance card(s) and a Photo ID
*Completed New Patient Paperwork (click here

*Please note that our Physician Assistant cannot see patient with Medicare or Medicaid Insurance

New Patients, in order to finalize your registration process, please obtain a referral from your primary care physician and have the referral and medical documents faxed to Dr. Shogan’s office at (303) 322-0897 one week prior to your appointment. 

Please arrive 30 minutes before your scheduled appointment time to ensure all necessary paperwork has been completed. If your referral and medical documents are not received one week prior to your scheduled appointment, you will be notified that your appointment has been cancelled. 

**Please note, that our provider will not confirm your appointment until all documents have been received and your medical records have been reviewed.
